What Is A PCN?

PCNs are a key part of the NHS long term plan, designed to support and maintain the longevity of the NHS, with 1,250 PCNs established throughout the country. They are built on top of existing Primary Care Services and are devised to remove barriers between Primary and Secondary care. They therefore are better integrated with local health and community services, and we also work very closely with the North East and North Cumbria Integrated Care Board (ICB).

Our PCN is a group of healthcare professionals (doctors, nurses and many more) who work together to provide our patients with a wide range of personalised, comprehensive, and co-ordinated health and social care services. These services are offered in addition to what is already offered via your own GP practice.
