The NHS is under pressure and patients are often living longer, with long term health conditions. Patients now need more access to local health services than they ever have done before.
The PCN helps our 8 member practices by offering a wide range of personalised, comprehensive, and co-ordinated health and social care services. We work closely with other local organisation such as mental health, social care and pharmacy providers, and have a team which proactively manages elderly patients in care homes. Our appointments are often alternatives to a GP appointments because a surprising amount of patients need support which a GP is not a specialist in and the PCN has a diverse skill mix of staff with expertise which better fulfils the needs of these patient groups.
Working this way collaboratively at scale, means that GPs have more capacity to see their patients, have a wider variation of services to offer to support their patients, and ultimately, allows patients to be seen by the right member of the team at the right time.
